During the consult, your doctor will take a history from you about your past medical history, your eating habits and your activity level. Your doctor may choose to do perform some lab investigations to assess for medical causes for weight gain / loss as well. Additionally a biometric assessment is done for baseline evaluation and during all follow up review appointments.

 

Obesity & Metabolic Syndrome

Obesity is a key contributor to a group of conditions known as metabolic syndrome. It includes other conditions such as high cholesterol, high blood pressure and diabetes. With these chronic conditions, people are predisposed to developing many diseases including heart disease, fatty liver, kidney failure and peripheral vascular disease. 

Obesity is an international health problem and can affect both physical and mental health. It has also been shown to greatly affect self confidence and energy levels.

 

How overweight Am I?

Body Mass Index (BMI)

For adults, we use a simple calculation called the body mass index (BMI)

BMI = Weight in kg ÷ (height x height [in meters])

 

According to the WHO,

  • Underweight = <18.5
  • Normal weight = 18.5–24.9
  • Overweight = 25–29.9
  • Obese = >=30

 

 In Asian populations (who are in general smaller in size), we should use,

  • Underweight = <18.5
  • Normal weight = 18.5 – 23.0
  • Pre-obese= 23.0 – 27.5
  • Obese = >=27.5

 

Weight Loss Treatment

Obesity treatment must be approached in a multi-disciplinary manner , and requires willpower and determination from the patient. The patient’s will is usually the largest contributing factor to success.

In general it requires

  • lifestyle management
  • Eating habits and dietary changes
  • Regular physical activity
  • Adjunctive medications (as needed) to aid in the journey (not magic slimming pills!)

Orlistat/Xenical

Orlistat inhibits the absorption of fat into the body. The fat stays in the gut instead of being absorbed into the bloodstream. This medication is safe but the gastrointestinal side effects can cause inconvenience – oily stools, diarrhoea, gas, increased bowel movements.

Phentermine/Duromine

Duromine works by suppressing your appetite and increasing your metabolism. It comes with side effects including palpitations, insomnia, high blood pressure, anxiety, dry mouth and nausea. This medication may not be suitable for people with certain pre-existing medical conditions.

Semaglutide (Ozempic / Rybelsus)

Semaglutide’s belong to the GLP-1 agonist family of drugs and work by essentially telling our brains we are full. This results in reduction in overall caloric intake and ultimately – weight loss. It is marketed under Ozempic which is a weekly injectable and Rybelsus which is an oral tablet you take daily. This form of GLP-1 is currently only approved for Diabetic patients in Singapore and not purely for weight loss at this time.

Saxenda

Saxenda is the brand name for a prescription-only, injectable medication known as Liraglutide. It is also a GLP-1 agonist but is shorter acting then Semaglutide and required daily injections. If you are obese, or overweight with weight-related medical problems (such as high blood pressure, high cholesterol, or type 2 diabetes), this medication can help with weight loss and keeping the weight off. If you are currently on insulin for treatment of Diabetes, Saxenda may not be suitable for you.
